Important Flu Shot Information

No matter your role in our health network, we all share the same mission – to heal, comfort and care. To do your job to the best of your ability, you first have to take care of yourself. As flu season approaches, one of the best ways you can help our patients avoid influenza infection is to get a flu shot. As the community is approaching the 2016-2017 Flu Season, LVHN has determined that all Vendor Representatives and Employees who have patient contact must be vaccinated prior to November 23rd, 2016.

Patient contact is defined as ALL personnel who perform work tasks, routinely (e.g. RN, Technical Partner) or intermittently (e.g. HVAC mechanic, food service delivery personnel), within six (6) feet of patients, or who have contact with the patient environment (included all areas of in-patient and outpatient departments) in the performance of their job duties.

Those who do not have patient contact may decline the vaccine by submitting the declination form. You must contact Supply Chain Management at 484-884-0230 to receive the appropriate declination form.

You must be vaccinated or have submitted a declination by November 23rd or you will not be permitted to visit any LVHN facility. This requirement will be managed by LVHN’s Supply Chain Management division and VendorMate.

Where can I get vaccinated?

Vaccinations will be offered at Employee Health Services at Cedar Crest and Muhlenberg during walk-in hours. Listing of walk in hours can be found at http://supplychain.lvhn.org/ Important Flu Shot Information.

PLEASE NOTE: If receiving your flu shot at LVHN Employee Health Services, you MUST contact (email) Elizabeth Oberland at Elizabeth.Oberlander@lvhn.org. There is specific information needed prior to receiving your flu shot.
