New Vendor Registration

Every vendor representative doing business with Lehigh Valley Health Network must go through an annual credentialing program to be granted access to the LVHN campuses. The program consists of the following components:

  • Achieve the certificate of completion by passing the exam in the Credentialing Module
  • Meeting the standards for GHX Vendormate registration
  • Signing the LVHN Vendor policy and confidentiality statement (signed copies will be sent to GHX Vendormate)

What is GHX Vendormate?

LVHN has contracted with GHX Vendormate to manage the vendor representative credentialing program.

To register with GHX Vendormate:

  • Register at vendormate.com
  • GHX Vendormate will work with you in setting up your profile and obtaining the needed paperwork to complete the registration
  • Once approved, you will have access to LVHN campuses according to our policies

GHX Vendormate will work with you directly to obtain (via e-mail or fax):

  • Electronically execute Vendor Policy and Confidentiality Statement
  • Complete the Credentialing Module, complete your general exam for Certification of Completion
  • Proof of PPD testing and immunization record
  • Flu Vaccination
  • Digital head shot photo which will be printed on your visitor’s badge
  • MMR Vaccination
  • Varicella Vaccination
  • Clearance from the OIG and EPLS website (US government fraud & terrorist sites)
  • Product Service Competency Letter
  • HIPAA Training
  • Certificate of Liability Insurance
  • Additional credentialing is required for vendor representatives in the following areas: O.R., Cath & EP Lab, Radiology and Pharmacy

  • GHX Vendormate will proactively send you reminders of any expirations on your requirements (i.e., if your PPD is going to expire, they will contact you to obtain an updated copy)
  • GHX Vendormate holds your profile information in a secure database for your privacy
  • Once in the GHX Vendormate database you can use your profile at other participating hospitals

Important things to know…

*Active vendor representatives are required to be registered with GHX Vendormate to have access to conduct business on any LVHN campus

*An active vendor representative is any clinical or non-clinical representative visiting an LVHN campus
